Objetive: Determine the strength of association of shoulder pain in both groups in comparative. Methodological Design: Prospective Cohort. Stimation of sample size: We estimated a sample of 120 patients divided into two groups: Group I of low pressure pneumoperitoneum and Group II standard pressure. Results: We analyzed 138 patients divided into 2 groups: Group 1 low pressure pneumoperitoneum (12 mmHg.) with 64 patients (46.4%) and Group 2 standard pressure (15 mmHg.) with 74 patients (53.6%). The average age of both groups was 42.75 years, with female predominance in both groups (67.95%). No differences in anthropometric values of both groups, or in surgical time or hemodynamic variables. The shoulder pain was present in 55 patients in both groups (39.9% of subjects) divided in Group I of pneumoperitoneum in 13 patients (20.3%) and group II, 42 patients (56.8%) P = 0001. Odds Ratio (OR) of0.19 (95% CI 0.07 to 0.75). Conclusions: Low pressure cholecystectomy is associated with less shoulder pain compared with standard pressurized insufflation.
